a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Our mission and goal is to provide quality care and service that promote dignity and self-esteem to individuals with developmental disabilities. We are committed to serve these individuals as if they were our own family members. We strive to respect, protect individual rights of each person and the families we serve and ensure their full participation in the community.

b) Describe your agency’s staff orientation and training process.

Each new staff will be required to complete all board required training, knowledge of our company's policy and procedures, and any additional training specific to their job/caseload assignment. Staff will have on going training throughout the year to ensure certifications and skills in their areas.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

In efforts to meet the needs of the consumers, we will work with them and their team to place staff, preference, circumstances, and schedule will also play a role in making that match.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Our agency offers a very flexible schedule. Our flexibility is based on the services and schedule needs of each individual consumers.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Our company will provide each consumer with a routine staff, however our company has in place a pool of qualified staff that a consumer will be familiar with in case of a call off or other emergency.

f) Describe how your agency would address individual or family concerns.

Our company has a complaint process that each consumer, their family, and teams will be introduced to at their initial meeting. We will also provide regular surveys to ensure consumers satisfaction throughout a consumer's contract.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Our agency has set up and continuously build relationships with community organization to have regular activities involving but not limited to our consumers. Such as the fun bus, traveling zoo and other agencies. Activities are tailored to our consumers specific interests and need. Our goal is to encourage our consumers to live full active lives, enjoy and have fun exploring what they like, and become productive members of the community.

The Director/ CEO has a Ph.D in Special Education with over 26 years of experience teaching students with developmental disabilities of all ages in a classroom setting . She has a proven teaching career and was nominated by her administrator as the best teacher of the year in 2010. She has outstanding references from parents and professional colleagues. Now, she is committing that experience and knowledge to serving individuals with special needs on a larger scale. Our agency encourages our consumers to develop daily life skills, train for jobs, and maintain participation in the community.

Christies Community Connection offer on-going trainings to all staff, work shops, monthly staff training, certifications, guest speakers on various topics beneficial to our consumers are invited, staff are continually updated on new laws and regulations.

We offer variety of activities based on individual interest, and planned weekly schedule. We strive to engage our consumers in indoor/ outdoor activities that are fun, meaningful, and would enhance their academic/social skills.

Our hours of operations are 9:00AM-3:00PM Monday through Friday. All federal holidays are observed.

i) Describe a typical day for an individual who receives your agency’s day services?

All activities are highly individualized .Every consumer has a planned weekly schedule based on individual need and interest. When they arrive in the morning, they settle in for 20 to 30 minutes having snacks or just relaxing, then are assisted to their planned activities for the day. Our goal is to assist consumers develop vocational /social skills that would equip them to become productive and independent in the community. We offer structured and unstructured activities. Our program is consumer centered. Every individual is unique and services are highly individualized.
