a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.
We are a Medicaid funded agency who serves, Individuals with Developmental Disabilities. Positive Individual Living Options put much pride and dedication to enhancing the lives of individuals that we serve. Positive Individual Living Options team will assist clients to be the PIOLT of their lives to live in the way they wish to live with our ongoing assistance and support. We will be the co-pilots of their lives and ensure that they are heading into the direction that they wish to with our on-going support, assistance, and confidence to succeed in their dreams and future.
Principles of Individual’s Self Determination and Positive Culture
The principles of Positive Individual Living Options Team for Self Determination and Positive culture are the values that our company and employees will follow to ensure that each Individual we service live the quality of life that is important to them. We believe in people first, we believe that each individual has the potential to live the lives that they wish to live. We encourage family participation and community integration. We believe in supporting and encouraging self advocacy for all individual we serve.
Each individual will be encouraged to choose where they live, how they live, and at what level of assistance they will need to achieve their personal life goals. We believe in self advocacy.
Each individual will be have participate in developing a unique life plan including needs and desires and what level of assistance they will need from our organization to assist them to achieve the vision they have for their life. We will assist them in achieving their life dreams including support and guidance.
We are obligated to use Medicaid funded waiver services in the most effective and efficient way. We have a responsibility to ensure that the services and supports we provide will ensure the quality of their own lives bases on their choice and their own life vision.
Positive Individual Living Options team will work with each individual and their team to create a positive culture in which they choose to live. Encouragement of healthy relations within the family and the community will be provided. We will encourage the individuals we serve to making positive decision related to their lives and assist them in pursuing their ideal life. We will encourage community involvement and positive choices for a healthy and fulfilling life for all.
b) Describe your agency’s staff orientation and training process.
All staff members will undergo required training, which will be determined by the position for which they are hired. During all new hire orientation, all employees will receive a minimum of 8 hours of training including but not limited to: Overview of serving individuals with developmental disabilities, Rights of individuals, Overview of basis principals and requirements for providing HCBS services, MUI training, and Universal precautions, Universal precautions, and MUI, first aid, CPR, Home fire and severe weather training, medication administration and side effects (specific to Consumer). Staff will also be required to complete any additional training set forth by the Lucas County Board of DD. All staff will be trained annually on the above stated trainings. In addition, Positive Individual Living Options Team employees are required to receive mandatory throughout their employment. Trainings will include but are not limited to: Consumer rights/protection, Abuse/neglect/withholding services, conflict mamangement resolution, therapeutic holding when allowable and personal protection. Employeer completes BCI check every 3 years, driving abstract annually, and all employees provide employeer with proof of autombile insurance monthly.
c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?
Management selects the individual's staff member based on needs and personalities. Management attends the initial start date with all new individuals and their staff member. It is important to ensure that the staff and individuals whom we work with is a good working relationship. Positive Individual Living Options Team refers to this process as a getting to know one another meeting. It ensures that the individual and our staff members are comfortable with the staff match.
d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).
Positive Individual Living Options team is flexible with individuals staffing needs and we schedule according to needs of our individuals. We are aware that a schedules, appointments, and individual needs change.
e) Describe how services will be provided in the event of an agency staff illness or emergency.
Positive Individual Living Options team ensures that all individuals have a back up staff members who has been trained in the event that their primary staff member becomes ill.
f) Describe how your agency would address individual or family concerns.
Positive Individual Living Options team ensures that there is open communication between the individuals and their families as well as with the LCBDD. Managements address all concerns with both the individuals we serve and their families.

Positive Individual Living Options team provides all of the individuals we serve with community activities and encourages them to participate in activities that they show an interest in.

Positive Individual Living Options team’s philosophy is based on self determination and individuality. We provide one on one services to the individuals we serve based on their desire.
