Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Senior Centers Inc. is dedicated to providing a variety of programs and services to senior citizens and to individuals with disabilities. Our Adult Day program is very unique as it is integrated with our other programs at the communty senior center. We have a long history of providing quality programs in the community. We began a realtionship with the county Board of DD in 1994 as a pilot program and expanded this to offer Adult Day in our facility in 2006. It has grown into a successful integration program. Fully integrated opportunities are presented to consumers of all abilities with additional attention given to those with special needs.

b) Describe your agency’s staff orientation and training process.

The Executive Director ensures that each new Direct Care Worker is trained to work with all consumers served. Training includes specific topics on DD, behaviors, CPR/First Aid and agency paperwork.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

During the interview process the interviewee is introduced to the consumers he/she would be working with. The consumer is given the opportunity to give feedback to the staff.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Our agency's Adult Day service is open from 8:30 am till 3:00 pm Monday through Friday. We offer service to anyone in Lucas County.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Additional management staff of the senior center have been trained to work with the Adult Day consumers in the case of needed backup.

f) Describe how your agency would address individual or family concerns.

Staff scheduling and coverage plans are based directly on the number and needs of the consumers which can vary. Each consumer will have a staff person who is primarily assigned to work with him/her. This staff person has the responsibility for assuring that the consumer’s needs are met. Communication will be ongoing between staff, the assigned SAS, and the individual's family at bi-annual review meetings to allow for continuity and consistency in providing services. Daily communication logs are kept on individuals if requested.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Our entire program in working with the Board of DD has been to provide an opportunity for consumers to participate in programming for independent senior citizens. Fully integrated opportunities are presented to consumers of all abilities with additional attention given to those with special needs. Our programs offer an individual the opportunity to socialize with peers, participate in holiday parties, fun games, crafts, educational programming, medical screenings such as blood pressure checks and hearing screenings, field trips and transportation to outings in the community. All consumers over the age of 60 are eligible to use additional services at the center including transportation, volunteer programs, social needs programs such as food commodities, Farmer’s Market Coupon program, and daily lunchtime nutrition program.

All direct care staff and management receive training in the following areas: Overview of serving individuals with developmental disabilities; rights of individuals; overview of waiver services; major unusual incidents; emergency planning including CPR, First Aid and Severe Weather, fire drills; reporting responsibilities; universal precautions and infection control.

u) Please describe your agency’s internal system for quality assurance.

The Board of Directors at Senior Centers, Inc. conducts a periodic review of all programs and services at Senior Centers, Inc. to ensure quality programs are being delivered and consumers’ needs are being met. The Executive Director uses this information, along with the appropriate staff member to improve or change programs and services as necessary. A consumer satisfaction survey will also be conducted with the Adult Day service to assure all consumers’ needs are being met, following the Individual Services Plan. Other controls to promote high quality services include ongoing training programs in responding to safety and emergency situations.
Staff and consumers are encouraged and expected to suggest and recommend changes in programs. This will allow management to improve all services where needed. Input is also encouraged from the consumer’s family and friends as well as the community at large.

Outings include: movies, out to eat at local restaurant, community special events and areas of interest within a 60 mile radius. Examples include: Calder Dairy, local parks, Hayes Presidential Center, Manor House at Wildwood, Vietnam Memorial, car shows Snoops Dream Museum and the Toledo Zoo. We also offer shopping to local stores on a regular basis.

A wide variety of programs and services are offered at the center. Activities include educational speakers on a variety of topics, entertainment programs, parties, health promotion activities, arts and crafts projects, bingo, Bunco, card games, board games, pool table, current events discussions, cooking projects, Wii gaming, health screenings and exercise classes. Educational instruction is also available in computers and ceramics.

Opportunities are available to assist other individuals with serving lunch, setting up lunch tables, decorating for parties, playing games with others and shredding documents.

Individuals are encouraged to participate in all activities and educational programs that are offered at the center, available to all the seniors attending our center. Our programs offer an individual the opportunity to socialize with peers, participate in holiday parties, fun games, crafts, educational programming, medical screenings such as blood pressure checks and hearing screenings, field trips and transportation to outings in the community. All consumers over the age of 60 are eligible to use additional services at the center including transportation, volunteer programs, social needs programs such as food commodities, Farmer’s Market Coupon program, and daily lunchtime nutrition program.

Our service includes 2 full time staff and one part time staff person, operating 5 days a week for 6 1/2 hours a day. We currently serve 8 individuals in our day service program, both part and full time. Our program offers fully integrated opportunities to consumers of all abilities, offering the individual the opportunity to socialize with peers, participate in holiday parties, fun games, crafts, educational programming, medical screenings, field trips and transportation to outings in the community.

h) What are the typical hours of your agency’s services/supports?

Our agency's Adult Day service is open from 8:30 am till 3:00 pm Monday through Friday.

i) Describe a typical day for an individual who receives your agency’s day services?

A typical day starts out with coffee and a current events discussion. Every day the individuals have an opportunity to participate in the scheduled center activity for the morning which may include an educational speaker, blood pressure screening, arts and crafts or an outing. An exercise walking program is also offered most mornings. Lunch is served every day at 11:30 with clean-up to follow. The afternoon may consist of playing cards, Uno, Skip-Bo or a favorite board game. Other opportunities include scheduled Bingo, Bunco, special parties or entertainment, movies or playing the Wii game. Once a week an exercise program is offered that includes relaxation and yoga and nutrition counseling.
