Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Shining Star Residential Services, LLC believes in caring for the whole person.
We focus on building relationships, fostering independendence and assisting our clientele to "reach for the stars" in their individual growth. Our mission is to provide quality care to those in our charge and to assist our individuals to be the best they can be.

b) Describe your agency’s staff orientation and training process.

All staff will be assigned a supervisor for each home. The home manager will monitor the overall performance of the employee. Also being monitored is the interaction with the individual assigned. All staff will be trained prior to working in the home. All new staff will be required to have a minimum of 12-16 hours of training prior to working with the individuals by themselves. Focus will be on building rapport, knowledge of individuals ISP/BSP and any medications or medical instructions. All staff will receive performance reviews.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Staff recruitment will begin with respect of guardian/client requests for particular individuals i.e. female or male staff. All efforts will be made to accommodate the requests of the guardians and clients when possible. All new staff will receive training in the following: CPR/ First Aid and blood –borne pathogens, medication administration and its’ side effects. Training on the behavior plan/individual service plan (of client assigned to), crisis prevention training, MUI training, and a review of company policies and procedures.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

We work in conjunction with Service and Support Specialist to come up with a schedule conducive to working with the individual. We will work in conjunction with the ISP in regards to schedule.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Staff will be assigned to clients per agreement of the ISP. When there are absences, a relief staff will be called to cover the house. Consumers will be informed of staff scheduling and any possible changes when possible.

f) Describe how your agency would address individual or family concerns.

We would speak to the individual or family about the concern. We would the family/individual an opportunity to correct matter if possible. If not, we would reassign the employee if necessary. It is our philosophy to solve all concerns at its earliest stage.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

We at Shining Star Residential Services believe in including the individual in as much community inclusion as possible. We send out calendars of activities monthly that outlines the various activities we are a part of. The individual has a voice in the things they want to be involved in from People First to Rec Jams ect;

We offer periodic training for the staff throughout the year. We also offer managerial training for supervisors. All employees are required to have a minimum of (8) eight hours of training. We usually exceed that number with the various training throughout the year.

ON A QUARTERLY BASIS, AN INTERNAL MONITORING OF SERVICES WILL TAKE PLACE. FOCUS WILL BE ON THE FOLLOWING: PERSONAL INCOME, HOUSING, PERSONAL SATISFACTION, HEALTH AND SAFETY AND THE EFFECTIVENESS OF THE INDIVIDUAL SERVICE PLAN (ISP). THE TEAM WILL WORK MAKE IMPROVEMENTS, RECOMMENDATIONS AND FOLLOW-UP.
