Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

C & J Tender Heart Residential Services is a family oriented team of trained professionals committed to making sure that the individuals we serve have the tools they need to reach their full potential while maintaining their independence. Our loving and compassionate team will strive to provide excellent support and care to enhance lives and achieve new goals. Our services are customized according to the individuals care needs.

b) Describe your agency’s staff orientation and training process.

Staff will receive extensive training in orientation and also a yearly in-services. The orientation as well as in-service training will address Residence rights, Compassionate Care, MUI's and Incident Reporting, Support Services, Active Treatment. They will also receive First Aid CPR prior to providing services.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

C&J Tender Heart RS management team will do a pre-screening according to the clients wants and needs for the staff who will be working with that individual. We want our services to be customized for each individual so we will use the information from the interviews for staff selection. We will then select a compatible staff (for example: a younger individual who is into sports may want someone around their age who is also interested in the same thing) to work with them. If this is not who they would like to work with they can interview team members available and select the team or member they would like to work with.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

We are currently providing services in lucas and wood county but are open adding other locations. We are very flexible with areas and hours. We have all shifts open and are open to providing 24hr staffing if needed.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

For all individuals a member of the management team as well as alternate staff will train with every individual so that in the event of an emergency there would be someone to provide the needed services when allowed.

f) Describe how your agency would address individual or family concerns.

Any individual or family concerns would be addressed immediately. C&J Tender Heart RS would do anything within our power and reason to accommodate the individual or family request.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

The Tender Heart time uses the free an reduce cost amenities in the community for life enrichment. We also provide transportation and group activities to encourage involvement.

My team has work in this field for many years. We have worked with many different diagnoses and have learned that each person is unique and are affected differently. With that in mind, we do not proclaim and area of expertise but offer a services to those who can benefit from the areas of services we provide.

Staff will receive ongoing training from the agency as well as taking classes availabe at the Lucaus County Board of DD

We will continuously monitor and look for ways to improve our services. Within 30 days of service will contact the individuals and families to assure we are providing the services needed and requested. After 90 days a full evaluation will be given to critique services and modify or make changes were needed.

We want the individuals we serve to live their life to the fullest. We want them to be happy, healthly, and successful while being independent. We will give them the tools, training and assistance they need to do just that. We will customize a plan that will work for the individual and their families.

We provide group activities, dating services, field trips and vacations. Our services are catered to the individuals we serve so group outing could be within the company or anywhere in the community.
