Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

CRS's philosophy rests on its mission to provide lifestyle support to persons in Northwest Ohio with Developmental Disabilities. CRS fosters greater levels of independent living, community inclusion, and personal empowerment. CRS is a non-profit agency.

b) Describe your agency’s staff orientation and training process.

CRS ensures staff are trained in all areas required by DODD rules and regulations. Additionally, selected trainings are required for staff that are not mandated but are necessary for quality provisions of services in areas such as Community Inclusion, Personal Choice, and Gentle Teaching series. Several days of one to one shadow training of supervisors and staff following training on individual specific plans.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

CRS is committed to ensuring staff work well with the individuals they serve ensuring respectful relations. Management has worked to resolve any conflicts with individuals in whatever manner in necessary. CRS performs bi monthly planning sessions from the home up thru management levels to ensure quality services are delivered per the individual's satisfaction.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

CRS is a 24 hour service agency. As an agency with Licensed and Supported Living (24 hours a day seven days a week) and Day Program (Monday thru Friday, 9a-3p) services are offered. CRS services individuals in Lucas and Wood County.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

CRS ensures staffing is provided per the individuals needs. We provide trained staff to fill shifts in cases of primary staff illness or emergency.

f) Describe how your agency would address individual or family concerns.

CRS first choice is to offer meeting to resolve issues or concerns. We do have a formal grievance procedure, when necessary to formally approach resolution at the upper levels of the agency.

CRS ensures opportunities are presented to individuals per their desire and makes all efforts to engage individuals in the surrounding community.

CRS has a long standing history of serving individuals successfully in Licensed setting for 35 years and in Independent supported living for nearly 30 years. Most of the individuals we serve have been long term with CRS services. Our expertise concentrates on providing quality level services.

At the time of hire as required, also an annual questionnaire is given to staff for feedback of what areas of training they feel would be beneficial. Non Required Training and education is arranged based on staff interest and or management decisions.

CRS assures consumer satisfaction and quality services through monitoring the functions of services by performing individual and agency planning sessions in the home, having staff team meetings, holding management meetings with all personnel, performing annual personnel evaluations.

CRS encourages those who are interested in services to make visits to the locations under consideration to meet staff and observe services.

Individuals enjoy a variety of arts and crafts, leisure recreation activities, and at least one community outing per week on a routine basis. Individuals participate in the decision making of the activities and trips they enjoy, so services are tailored to individuals choice.
