Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

We specialize in providing individualized care for every individual. Every client is unique and requires individualize client centered care. Our staff is highly qualified and provides the care that every client and family desires. Our focus is client centered care.  This allows every client to receive individualize care for their specific needs.

b) Describe your agency’s staff orientation and training process.

Staff of agency providers must complete eight hours of training in accordance with standards established by DODD. Agency is prohibited from providing services, until certification is obtained from DODD. All Agency’s employees and independent provider’s and independent contractor’s must:
-Be at least eighteen years of age
-Hold a high school diploma or GED
-Have a valid social security number and one of the following forms of identification:
-CPR & First Aid
-State of Ohio Identification;
-A valid driver’s license; or
-Other government-issued photo identification
A current report from the bureau of criminal identification and investigation (BCII), which demonstrates that he/she has not been convicted of or pleaded guilty to any of the offenses listed in division (E) of section 5126.28 of the Revised Code. If the subject of the background investigation cannot present proof that he/she has been a resident of Ohio for the five-year period a FBI and BCII investigation will be required immediately prior to the date of the background investigation, he/she shall request that BCII obtain information regarding his/her criminal record from the federal bureau of Registry. Agency will complete pre-employment exclusions check and at least every 5 years thereafter. A BCII report shall be considered current if it is not more than one year old.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Individuals with disabilities have the freedom to plan their own lives and to pursue things that are important to them with the support of independent planning and support coordination. Individual's have the freedom to choose and set up the support they need to pursue the life they envision. Our Agency will support the individual and provide the services in accordance with the ISP. The Agency will make every effort to accommodate the individual's preference with Agency staff.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Our Agency will provide availability and flexibility to meet the individual's needs to maintain compliance with the individual's service plan. The individual's needs will be our Agency's top priority. Our Agency will provide daily care with flexible hours to all areas of Lucas County.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Our Agency will ensure that in the event of an agency staff illness or emergency, alternate coverage will be provided. Agency will make every effort to stay in compliance with the individual's service plan and provide the necessary coverage. The individual will be notified of a change in Agency staff. In the event that no available staff would be available, appropriate contacts including the individual, authorized family, POA, Guardian or parents will be notified as well as support coordination.

f) Describe how your agency would address individual or family concerns.

Agency will promote and maintain individual satisfaction with services delivered. Individual/Guardian will receive a verification of services phone call quarterly or as needed. This call will ensure the individual's satisfaction is being met for services delivered. Individual/Guardian will also have phone number of CEO for any complaints that need to be addressed and or resolved.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Individuals with disabilities should have the freedom to plan their own lives and to pursue things that are important to them with the support of independent planning and support coordination. Every client is unique and requires individualize client centered care. Agency's care should be centered around the individual's preferences as supported by the ISP. Agency will promote increasing community inclusion and involvement for all individuals served.

Agency requires direct service staff to complete eight hours of training in accordance with standards established by the Department. Continuing education will be ongoing with the annual 8 hours of training, current CPR & First Aid. Agency will ensure compliance with initial and ongoing training requirements.

u) Please describe your agency’s internal system for quality assurance.

The Agency will identify the areas in need of monitoring and evaluating. As a new Agency, this will be an ongoing process subject to change as needs to be address arise.
Monitoring and evaluation help improve performance and achieve results. More precisely, the overall purpose of monitoring and evaluation is the measurement and assessment of performance in order to more effectively manage the outcomes and help to develop results. Agency owners and managers have a responsibility for developing the staff effectively, ensuring that they receive regular quality supervision, clearly stated expected work standards, ensuring training and development opportunities are identified, facilitated and evaluated.
