Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Assured Health Services provides services to individuals with developmental disabilities that are built upon positive outcomes focusing on self-determination and personal development. By supporting individuals and being involved in the community we believe all people will grow and benefit personally.

b) Describe your agency’s staff orientation and training process.

All staff members undergo initial training prior to providing service and will continue to receive training through out the year.Trainings will include but are not limited to: consumer rights, abuse and neglect, conflict management and resolution, and additional self-determination principles. Upon hire, all employees will receive a minimum of 8 hours of training including but not limited to: Overview of serving individuals with developmental disabilities, Rights of individuals, Overview of basis principals and requirements for providing HCBS services, MUI training, Universal precautions, first aid and CPR.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Each individual is involved in the decision process.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Assured Health provides services through out Lucas County. We are available and provide services 24 hours per day, 7 days per week.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

We are available 24 hours per day, 7 days per week. In the event of an emergency back up staff will be contacted to replace existing staff.

f) Describe how your agency would address individual or family concerns.

We have a formal grievance process that is open to all individuals, families, staff and community. We follow the process through to completion to assure that each concern has been resolved and to limit such concerns from reoccurring in the future.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

We believe all people should be involved in their community as much as possible. We encourage community involvement as often as possible. We have monthly calendars with group outings and each individual has a voice in the activities they want to be involved in.

Assured Health places an emphasis on professional development. We have regular staff meetings and require additional trainings, workshops, and seminars to keep staff skilled, knowledgeable, and growing personally and professionally.

We have a thorough policy on internal Quality Assurance and Quality Improvement. We perform a number of checks and balances that ensure proper supervision of the services we provide. Compliance, open communication, transparency, and proper supervision is the foundation of the services we provide.

Assured Health maintains sufficient staff to meet the individual needs and services. We are available 24/7 and offer services as desired based on each individual's ISP.

We offer regularly scheduled trips to places such as the Art Museum, the Zoo, Mud Hens games, Walleyes games, the Botanical Gardens, and other events. We also have a guy's day and a girl's day each month.

We have various positions available including yard work, laundry, cleaning offices, working with dogs including giving dogs baths and janitorial work.

Assured Health is committed to working within the community. We feel the city of Toledo offers many wonderful opportunities for its residents and we take advantage of everyone of them. Our goal is for you to enjoy all the community has to offer while being surrounded by friends who want to do the same. We offer various activities and outings in the community, such as trips to the park, Toledo Zoo, Humane Society, sporting events, shopping, nature hikes, and many other activities that will keep you excited and entertained!

We offer multiple recreational and leisure activities such as playing with dogs, playing video games, doing puzzles, painting, watching movies, computer activities, nature walks, playing games outdoors and many more options to meet your preferences.

We have volunteer opportunities at several local businesses and community events. Each volunteer opportunity is based upon personal preference and personal strengths. We promote an active lifestyle. At our Thrive Center we encourage people to be themselves. We offer many opportunities for people to grow, have fun, and to shine! Services include transportation, recreation and leisure, skill development, paid work, personal care, community outings, games, and socializing with friends.

Everyday is new and exciting. We provide a structured, balanced, and flexible environment. There are many different opportunities that are provided every day for people to be themselves and shine!. We have a monthly calendar with daily activities listed so you can prepare and look forward to upcoming events.
