a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Beacon of Light, Ltd. firmly believes that our greatest resource is our individual and collective commitment to the physical, mental and emotional well being of our clients. Beacon of Light, Ltd. provides individuals with the support, guidance, and motivation necessary to achieve individual goals, personal growth and greater independence. This is achieved by staff working as a team, to inspire and promote enhancement of skills, both in the home and the community, while fostering self-esteem, productivity and the optimum quality of life.
We are committed to the unceasing pursuit of a culture of total quality by focusing all of our activities on strengthening the organization so that we may better serve our consumers. b) Describe your agency’s staff orientation and training process.

We have a thorough and comprehensive process that considers all the needs and safety of the individuals to whom we provide services. We submit to state review and accountability. Our staff are trained in all areas of the ISP as well as Behavior Support training when applicable.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Individuals are encouraged to have a "meet and greet" prior to actual services so that they meet the staff that will be working with them. In addition, they are able to communicate with the office and request a change in staff at any time. We follow-up any time a new staff is assigned to a consumer to ensure that it is a good fit and there are no concerns.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Our services are dependent on the customer's needs and support because every person's schedule is unique. We understand the need for flexibility and we make every effort to accomodate the individual's support plan. We can plan on a monthly basis or a week to week basis.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Typically, we like to train a back-up staff initially to ensure continuity of services. This also puts the consumer at ease knowing that they have already met and are familiar with the staff.

f) Describe how your agency would address individual or family concerns.

Beacon of Light, Ltd. conducts consumer satisfaction surveys on a regular basis and welcomes feedback so that we may improve our services and be aware of the consumer’s needs and wishes in an effort maintain a good relationship with each and every consumer. We understand that communication is key in any relationship. We also understand that family and/or guardian satisfaction and participation is just as important as the consumer satisfaction. We know that only when we work together as a team can the consumer get the best possible outcome. We know that we are there to build a relationship, but also to do a job. Part of doing our job is respecting boundaries when it comes to guardian and family involvement, as well as other providers.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

We are a part of several local community organizations that keeps us informed of opportunities to involve individuals. We prepare a monthly calendar of local events that we mail to each of our consumers so that they are aware of available events. We plan group activities that are open to all consumers. We research specific areas of interest and provide the information and/or planning necessary for the consumer to pursue that interest.

We provide homemaker personal care services, adult day services, respite services, non-medical transportation, and supported employment services. We specialize in dealing with consumers with behaviors.

Beacon of Light Ltd. is certified to do Medicaid state plan home health aide services as well as nursing, physical therapy, occupational therapy, and speech therapy. Our staff of highly qualified aides, nurses and therapists can assist our clients in their medical recovery.

We provide continuous education throughout the year on individual service plans, medication admnistration, and CPR. In addition to these service related trainings, we provide trainings on leadership development, Autism, non-medical transportation, and management styles and techniques.

u) Please describe your agency’s internal system for quality assurance.

We have house managers at the direct support level as well as the management level. Documentation is checked regularly to make sure it is in compliance and that all elements of the ISP are being delivered satisfactorily. Management performs regular consumer surveys as well as random checks on employees during service hours. We submit and comply with all local, state, and federal regulations and audit procedures.

Beacon of Light builds relationships with their consumers and assists them in their daily living in an effort to improve their quality of life. We work with our consumers and the important people in their lives to ensure that they are happy and satisfied with their services. Beacon of Light staff are dependable and reliable.

We provide payee services for clients and we are currently listed as an agency payee with Social Security. We understand and have experience with benefit maintenance, payroll reporting requirements, and Social Security rules/regulations for payee services. Our payee services are provided Mon-Fri to ensure that emergency financial situations can always be handled. We provide all aspects of personal care, including meal preparation, housekeeping, and medication administration. We can assist with scheduling and transporting to medical appointments, filling prescriptions, and getting medical consults and logs completed in order to maintain an accurate medical history.
We participate in many different types of community activities such as Special Olympics, sporting events, video games, church events, concerts, exercise/working out, trips to the library, Cedar Point , zoo, spa days, shopping, movies, go-karts, and bowling - just to name a few! We provide our consumers with a monthly calendar of events to ensure they have a variety of activities to choose from. We can help you build skills in areas such as reading, gardening, computers, safety awareness, arts & crafts, basic jewelry making, nail art, cooking, money management, time management, grocery shopping, and laundry.

i) Describe a typical day for an individual who receives your agency’s day services?

It depends on the individuals needs/support plan. On a whole we start every day with general activities that reinforce community and group efforts. We provide a health regimen through training in our gym or written/verbal teachings. Individuals are given one on one time to reinforce their support plans and then there's lunch where they all come back together. Depending upon the activity or outing the day can still vary with the individual in mind.
