a) Describe your philosophy of providing services and supports to individuals with disabilities.

My goal is to offer opportunities to individuals with disabilities. Each individual, regardless of disability, has the right to experience a life of happiness, choices and freedoms. I look forward to assisting the individuals I serve to achieve this life, to support individual choices and encourage positive outcomes through a caring, supportive and positive environment.

b) Describe yourself and your background/experience with individuals with disabilities.

I am a licensed social worker with extensive experience serving individuals with disabilities. I have been a case manager in several agencies in the Toledo area and have also worked at The Lucas County Board of Developmental Disabilities as a job coach substitute. Professionally I have worked with disadvantaged youth, single mothers and fathers, individuals with terminal illness to name just a few of the populations. My interest in individuals with disabilities began when I was in high school as I volunteered with the "special needs" classroom (that was the official title at that time). I learned quickly that the students I worked with were just like any other students, just with varying needs. This knowledge led to me to want to assist individuals to achieve their fullest potential. As I worked with other populations, my interest always led me back to those with disabilities. In addition to Lucas County Board of Developmental Disabilities, I also held a position at Sunshine Children's Home as direct care staff.

c) Describe any specialized training or education you have received to prepare you to work with individuals with disabilities. Describe any areas of expertise.

I have a Bachelor's degree in Social Work and am a Licensed Social Worker. I have completed all requirements as defined by Lucas County Board of Developmental Disabilities to obtain provider certification. In addition, I have completed various courses in conflict resolution, non-violent crisis intervention, and work very effectively with individuals with behaviors.

d) Are you certified by the Ohio Department of Developmental Disabilities to provide medication administration?

e) Describe your availability and flexibility in scheduling services and supports for individuals with disabilities. (Days and hours available, areas of Lucas County you are willing to provide services, etc)

Preferred availability is Monday through Friday 7am through 6pm. However, I can be flexible based on individual needs.

k) Describe how services will be provided in the event of your illness or emergency.

I will provide services unless any illness prevents me from effectively providing quality services or if any illness is communicable. In the event that I am unable to attend, I will contact the appropriate person/staff immediately to make necessary arrangements. I am in the process of applying for agency certification and at that point, staff will be cross trained to cover emergency situations.

l) Describe how you would address individual or family concerns.

I will always maintain open communication with all parties involved. I acknowledge that both the individual and/or family concerns are important and will be considered immediately upon notification.

m) Describe specific strategies for increasing community inclusion and involvement for individuals served.

Based on individual interests, I would organize community experiences specific to areas of interest. In the event that an individual had any interest would require accommodations, I would work closely with any parties involved (the business, the organization, etc) to ensure accommodations would be arranged to offer the individual the same experiences as the general public.

I am confident in my abilities as a provider as this has been a field that I have always been passionate about. I am a firm believer in the rights of individuals with disabilities and will always advocate, assist and support this goal for any individual I serve.
