a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.
All Can Truly Serve is a day habilitation program for “adults with capabilities” in the Toledo, Ohio area. Through our programs, our goal is to create community enrichment for our "client Volunteers" and the organizations that we are involved with. Jerry developed the company in order to serve a population that has a lot to offer the world, but few opportunity to demonstrate their capabilities. All Can Truly Serve provides local non-profit organizations in Toledo with over 300 hours of service each and every week. Through our programs, we’re able to provide “adults with capabilities” the opportunity to work within the community, find their role within the community, interact socially with their peers, develop new skills, share their knowledge with others and gain real world skills that they can transition into jobs.

Our agency fully trains staff per state requirements, plus the following;

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?
d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).
All Can Truly Serve is available and flexible in a number of ways. We provide transportation to and from each individual's home. Each individual can volunteer whatever days they choose so as to accomodate their schedules. Individuals also have say in where they volunteer.
e) Describe how services will be provided in the event of an agency staff illness or emergency.
We have on-call staff to cover in any staff shortages.
f) Describe how your agency would address individual or family concerns.
All staff are approachable in nature, and individuals can voice any concerns. Individuals and family have access to Jerry Silveous, executive director, through phone or email to express concerns. Also, families and individuals may communicate with Service and Support Specialists who in turn relay messages to Jerry Silveous. When a concern is brought to attention, all efforts are made to remedy the situation.
g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.
Since our program is 100% community based, everyone involved benefits. Through this act, our volunteers are able to participate in the community and acquire new work skills, while also developing beneficial social skills that they can utilize in their daily activities. At the same time, our non-profit organizations receive the help and assistance that they are in need of. It's a win-win structure for everyone involved.
The basic structure of our program is to enrich the lives of our volunteers and those in the community through our volunteer program. In our volunteer program, adults with capabilities go out into the community and assist non-profit organizations with work and tasks. Just a few of the non-profits that our volunteers work at include the Humane Society, Camp Courageous, Ronald McDonald House, Metro Parks and local food banks.

All Can Truly Serve's area of expertise is the fact that it is an all-community inclusion program. 100% of tasks and activities take place at churches or food banks or other non-profit organizations. As such, each daily experience is natural and authentic as opposed to a segregated workshop setting.
Staff working for All Can Truly Serve are also experts in the field. Jerry Silveous is the “hands-on” Executive Director of All Can Truly Serve. He brings over 25 years of experience in the field to the organization. He has worked in direct care, as management, and also helped start two other companies in the field. Other Day Program staff offer many years of experience working with this special population.

Annual Staff Training Requirements (training received within previous 12-months)
All employees will annually complete training, in accordance with standards established by the department, in the provisions of governing rights of individuals set forth in section 5123.62 to 5123.64 of the Revised Code.
All employees will annually complete training, in accordance with standards established by the department, in the requirements of rule 5123:2-17-02 of the Administrative Code relating to incidents adversely affecting health and safety.
All Can Truly Serve believes that ongoing training and education beyond the orientation period is essential to provide the services our clients need. Participating in training and education is part of the yearly performance evaluation.

POLICY: Consumer Satisfaction
All Can Truly Serve, Co. will ensure that an annual survey is administered to all consumers and/or primary caregivers who have accessed services through this agency. It is our goal to complete a survey at the time of discharge from services or annually, whichever comes first. The Consumer Satisfaction survey will be used to determine satisfaction with our program.
Additional assessments and outcome measurement tools may be used to gather
information about our client satisfaction in regards to the services they received and to
gather data for our outcome measures for future grant funding.

All Can Truly Serve definitely offers individualized attention. In many ways, we are like a family. The bonds that form amongst the team are strong. Those that volunteer with us and give of their time and talents are fulfilling a purpose they have. Annually our Day Program attends the Fulton County Fair. Daily 3-4 small groups go to various locations in the community and volunteer.

Our recreational outings program is designed so that our clients assist in developing the calendar of activities. This program provides clients with the opportunity to participate in events in the community; have social interaction with peers and have fun while being part of a safe environment. A few common activities in the recreational outings program include bowling, miniature golf, going to the movies, out to dinner, picnics and taking hikes.

We are a 100% community based volunteer program. Weekly, we work at The Toledo Area Humane Society, Camp Courageous, Seagate Food bank, and Toledo Area Metro parks to mention a few. We also make lunches and deliver them to The Beach House of Toledo every week. Our program focus is on building work skills through community service and volunteering. If you would like an interview or take a tour for more info, please let us know. We currently do not offer paid work, but for those between employment or working part time, our program is a great community resource.

All Can Truly Serve provides non-medical transportation to and from all volunteer sites. Staff are extremely knowledgeable and experienced and serve to meet supports to all individuals according to each ISP. While ensuring safety and supports, staff also engage individuals in projects and work. Participants of our unique program visit a variety of organizations each week and therefore gain a breadth of skills and also the interaction with many people who impact them and also are impacted by them.

Lucas County, Ohio. We are %100 community based. Our Day begins in Oregon at 8:00am.

Monday thru Friday 8:00am to 1:00pm. Plus Transportation

Program begins at 8:00 in the morning. Everyone is assigned a staff and location to volunteer at for the day and crews get going. Location choices are at The Toledo Area Humane Society, Camp Courageous, Seagate Food bank, and Toledo Area Metro parks, Ronald McDonald House to mention a few. Routes home leave at 1:00 pm.
