Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Careology is committed to providing the highest quality of care and customized services and transportation to individuals with developmental disabilities. Careology is dedicated to maintaining the dignity & independence of each individuals in the comfort and safety of their own homes and in the community.

b) Describe your agency’s staff orientation and training process.

Careology orientation and training program exceeds the state regulations as an agency provider. The needs of each individual is carefully assessed, diagnosed, understood & met through the selective assignment of qualified, skilled, and trustworthy personnel. Based on Individual's needs, Careology first match the right caregiver/staff. Each staff member is mandated to complete 15+ hours of training prior to providing services. Biweekly meetings are held to ensure staff are up-to-date and complaints with all rules and regulations.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

The needs of each individual is carefully assessed, diagnosed, understood & met through the selective assignment of qualified, skilled, and trustworthy personnel. Based on Individual's needs, Careology first match the right caregiver/staff.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Careology is committed to providing services and supports around the clock, which extends beyond normal business hours and we can be reached 24/7 at (419) 973-2259 in case of an emergency or overnight support. All staff members are trained and counseled for any changes that are necessary to meet the required care.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

During the initial assessment, Careology ensures that there will be always a proper backup for each individual. Careology mandates other staff members to shadow for a few days to establish a relationship and recognize the needs for a smoothly transition, if needed.

f) Describe how your agency would address individual or family concerns.

The safety and satisfaction of individuals are our top priority. Any concern is always given full attention and consideration. A prompt and immediately action is taken by management. All staff members are trained to relay concerns, questions suggestions to case managers. After ensuring that concerns are addressed and then met, a preventable measures are established and a frequent follow-up is conducted by case managers.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Careology strives to increase community inclusion and involvement by engaging in multi-lateral community events. Many of our staff are heavily involved in the community, e.g., CedarCreek Church, The Free Community Clinic Services that is held in Downtown Toledo every Monday night, SSFAC non-profit and others.

All staff members will annually complete training, in accordance with standards established by the State, Careology, and Department in the provisions of governing rights of individuals set forth in section 5123.62 to 5123.64 of the Revised Code as well as the requirements of rule 5123:2-17-02 of the Administrative Code relating to incidents adversely affecting health and safety. Careology believes that ongoing training and education beyond the orientation period is essential. Participating in training and education is part of the yearly performance evaluation.

u) Please describe your agency’s internal system for quality assurance.

Daily monitoring of each individual's desired outcomes and supports. ISP for each individual is examined/reviewed frequently to remind staff members of each need on one-to-one basis. Surveys are sent out periodically to individuals and case manager address such surveys and take proper measure, accordingly.
