Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Here at Caring & Compassionate Healthcare, our philosophy is treat others as you would like to be treated. We feel this is especially significant to keep in mind/carry out, as we provide services within our community. We strive to go the extra mile for our employees, consumers, and families we serve. It is important that our staff encourage independence and respect the ideations and rights of consumers.

b) Describe your agency’s staff orientation and training process.

All employees of Caring & Compassionate Healthcare are trained in all areas required by the state. These requirements include, overview of serving individuals with developmental/intellectual disabilities including implementation of individual service plans, role and responsibilities of direct services staff with regard to services including person-centered planning, community integration, self-determination, and self-advocacy; universal precautions for infection control, including hand washing and the disposal of bodily waste; individual rights and how to apply daily for our consumers, dignity and respect (principles of the positive culture initiative), privacy, decision making (principles of self-determination, consent for services and treatment ), advocacy (relationship to guardians and personal representatives), financial management (relationship to payees) and most importantly confidentiality. Our employees are also trained on categories of major unusual incidents (MUIs), reporting responsibilities, trends and patterns of MUIs and unusual incidents, immediate response, prevention plans, the role of the investigative agent, Abuser Registry, and Health and Welfare Alerts. In addition, it is required each employee is CPR/ First -Aid Certified.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Prior to pairing an individual with staff we will offer face-to-face greeting with our staff. We will also work with families and the SASS to pair each individual up with someone who enjoys the same interest as they do. Each individual should feel secure that their opinion and feelings will be kept confidential and matters. If there is ever any dissatisfaction with staff we will not hesitate to replace that employee.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Our office hours are Monday-Friday from 8 AM-5 PM, with on call 24-hour management available , including holidays. Our scheduling for services is flexible and is based on the needs of each individual we serve.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

In addition to back-up staff, our management team will be on stand-by in case of staff emergency or illness.

f) Describe how your agency would address individual or family concerns.

We would set up a face-to-face meeting with all involved individuals (family/SASS), and work out a plan to resolve the issue. If a meeting setting is not an option, we can communicate through a confidential email or phone call if necessary.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

At Caring & Compassionate Healthcare we try to think outside the box and encourage low to no cost activities in our community. These activities range from indoor as well as outdoors, keeping each individual in mind in regards to their specific capabilities. We also will arrange group outings to promote social involvement as desired by each individual.

The agency’s CEO and Co-CEO are nurses and have experience in the field pre and post nursing careers. This allow us to have a holistic approach when providing services. It is important that we work in conjunction with clinical, medical, behavioral and personal support for the best outcome of individuals we serve. Combined we have 13 years of experience in various diagnoses.

We will provide annual and ongoing training through the local and state developmental disability entities. In addition, remain updated on diseases, diagnoses, and disabilities we encounter within our agency by providing staff meetings.

u) Please describe your agency’s internal system for quality assurance.

As we will strive to provide quality service we will follow up with families and individuals within 3-4 weeks of beginning services. We will also take suggestions from staff on how we can provide quality service in the community. We conduct quarterly evaluations by the individual and family on staff performance.

At Caring & Compassionate Healthcare we believe in a holistic approach when providing services and work together with families to ensure each individual reaches their full potential. We will offer personalized plans as we know each person is unique in their own way. We value the trust and opinions of the individuals/families we serve. Our main goal is to promote independence, provide a safe/secure environment that will give peace of mind to individuals and their families.
