Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Conteh Home Services, LLC is committed to the unceasing pursuit of, and commitment to, a culture of total quality, focusing all our time and energy on strengthening the organization, so that we may better serve our consumers. CHS is committed to filfilling its mission by performing all duties in a caring and professional manner. CHS recognizes that only through support and professional pride in our contributions will superior service be dalivered.

b) Describe your agency’s staff orientation and training process.

It is the belief and intent of Conteh Home Services (CHS) that all employees should be appropriately and thoroughly trained for the jobs and positions which they perform. If you believe that further training is appropriate and/or necessary for you to complete your job, you are encouraged to discuss the same with your supervisor. Continuous and on-going trainings to refresh and keep staff abreast of necessary requirements are strongly emphasized.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Conteh Home Services does involve our consumers , their guardians, family members and all interested parties in our staff selection process. All potential hires are encouraged to orient and meet consumers in thier homes with supervised approval from the office before actually hiring them.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Conteh Home Services is open to new placement with some reasonable availability. Our office hours are 9:00am - 5:00pm Monday through Friday. All supervisors are accessible by phone 24 hours to address consumer issues.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

We do have substitute and stand-by staffing protocol for all emergencies. In extreme situations home supervisors can fill in for regular home staff. In light of this we do train all supervisors as regular staff.

f) Describe how your agency would address individual or family concerns.

Conteh Home Services, LLC does have an open communication policy with family members, county board representatives and all other individuals of relevance to our consumers. We would prefer to resolve issues as adults and professionals. In extreme and difficult circumstances we request Special Team Meetings to handle disputes.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

We participate in various recreational and leisure activities within and outside the agency. Activities and participation are tailored toward specific interests to maximize benefits and satisfaction.

We engage in annual picnics, vacation trips and day trips on a 1:1 consumer-staff ratio. This process will maximize individual attention and care to effectively enhance health and safety.

We offer annual parties, trips, outings and recreational activities. Some of our trips have have been out of state.

Trips/visits to the parks, zoo, game parks, day trips and walks within the neighborhood. Some consumers participate in bowling leagues and bingo games.

The Day Program offers one social evening get-together for activities and snacks. We do have yearly picnics and trips to amusement parks and holiday resorts in rented cottages and /or rooms.

Our Day Program is activity based, reflecting the elderly consumers we presently serve. One transportation vehicle is available on site for outings and other leisure activities.

We are located at 330 South Reynolds Rd. here in Toledo.

h) What are the typical hours of your agency’s services/supports?

We are open from 9:00am - 2:30pm Monday through Friday. We observe all federal holidays.

i) Describe a typical day for an individual who receives your agency’s day services?

When consumers arrive they put away their lunch boxes and offered the opportunity to use the toilet. They are offered slight snacks. Depending on the scheduled day activity consumers are engaged until 11:00am when they eat lunch. Part of the programing may include walks to the near by convenient store or trips to the zoo, park or shopping malls.
