Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Our mission is to help aide and support individuals, in becoming independent and socially familiar with everyday task encountered in the home and as well as the community. While treating every individual equally and fair.

b) Describe your agency’s staff orientation and training process.

Here at Elite Residential Services we put our staff through a thorough and intense orientations, which includes going over the agencies policy and procedure manual and becoming familiar with the States rules and regulations. We also run a Bci and fbi check as well as abuse registry check. We makes sure each individual goes through a two step interview process with the CEO and owner, and then we make sure the individual can provide approved references that are non family. Once the interview process is over we schedule the employees for training through the state as well as first aide and cpr training paid for by the Agency. We then place the employee on a 90 probationary period where the staff member will be under observation and we also get feedback from our consumers.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Here at Elite while the direct care employees are on probation we allow them to work in the homes with our consumers, and while the staff members are in the homes the CEO will meet with the consumers twice out the month during the probationary period and get feedback on the employees and if there are issues we will discuss those with the staff member as well as the consumers and take action accordingly.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Elite residential provides services 24 hours a day 7 days a weeks 365 days a year, and someone will always be on call as well to answer any questions or concerns.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

We have staff members who are hired in solely as part time and full time employees, and we also hired contingent workers who are available on call for emergency purposes as well as fill ins for vacations

f) Describe how your agency would address individual or family concerns.

We will have monthly meetings with the individuals and there families with the CEO and owner privately to discuss and issues or concerns that wish to be discuss without the staff, then we will also hold a meeting with the staff and individuals and families if they wish to attend they are more than welcomed to discuss ideas suggestions and concerns.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Elite will plan trips weekly to get the individuals out the homes and involved in the community and allow them to interact in a public setting we will take our consumers to the movies on Tuesdays out to dinner to the local parks, church, gym, job fairs, basketball games and job fairs. And to any reasonable requested events.

Are specialty is getting our consumers out and involved in the community, to help them become familiar with every day task and socially comfortable. We want our consumers to look forward to being around people in the community and seeing different people places and things and helping them build a strong will for self inclusion and personal opinion, while allowing them to make their own decisions as to what they like and dislike and understanding that it's okay to have your own voice and help them understand their individual rights and that everyone should respect the choices they make rather others agree or not.

elite follows the mandatory training by the state for certain required training ex. mui, universal precaution are yearly however Elite does a refreshment course if a concern is brought to our attention and we also make sure our staff has med pass, cpr and first aide renewal courses yearly as well

u) Please describe your agency’s internal system for quality assurance.

Elite will do employee progress reports every six months that will be over seen by our House managers on the direct care staff to report to overhead the performance of the staff for example we will want to know how the staff interacts with the consumers and how the consumers respond and approve of the staff member, how often the staff member calls off or tardiness as well as other situations.

Elite would like families and individuals to know that we take pride in providing care to individuals and we understand that we are caring for your love ones and we know how much they mean to you, and that's why we plan to go above and beyond to make you all feel comfortable with knowing that you and your family made the right decision but most importantly that your family member is in a safe and loving environment at all times. We have an open door policy families are always welcomed in the homes and to call at anytime and speak to whomever requested, individuals have the right to change providers at any time if they are not happy with the services provided and no foul or ill feelings will be acceptable at anytime!!! Yes we have a monthly event schedule that as of now is set up by the owner and employees with different events we would like our consumers to attend such and basketball games Tuesday movies and dinner which we call a night on the town, and we also plan to take our consumers to the lights before Christmas and the malls.
