a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

b) Describe your agency’s staff orientation and training process.
Our First Love Residential Services continually has on-going training in order to stay within the regulations of the state and to best adhere to our clients needs.
c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?
We make it our responsibility to keep our clients happy. Staff members are placed with clients in order to accommodate with their needs, while offering the clients a choice to be cared for by another staff member if they choose to.
d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).
Our First Love Residential Services ore open a flexible to accommodate the needs of our clients 24 hours a day and seven days a week. We are willing to provide services to any area in the Lucas County.
e) Describe how services will be provided in the event of an agency staff illness or emergency.
In the event of a staff member being ill or in an emergency situation, we will provide a replacement staff to continue care for the individual.
f) Describe how your agency would address individual or family concerns.
With respect to the individual and their family, we would provide a meeting in order to develop an understanding about the concerns of the client. After, we would act in our best interest to fix the issue or provide the necessary information to ease the concerns and reassure the client and family that they are in fact in very good hands.
g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.
Our First Love Residential Services will continue to look for opportunities for individuals with developmental disabilities to show their talent and pursue their goals and interests in our community by educating small business owners, training facilities & recreational establishments of their many contributions to our society.

Our First Love Residential Services is certified to deliver homecare/personal care, HPC transportation, supported living, community inclusion, and personal assistance. Each of these services, we provide with love an honor, and with the best interest of our clients in mind.

We have 0 clients at this time. We recently opened our agency and have direct care staff ready to provide services when needed.

t) Describe your agency's plan for continuing education and on-going training for agency staff?
Our First Love Residential Services provide in house CPR & First Aid training, personal care & homemaker training and medication administration classes. Our staff is required to take Intro to DD, Specialized Support, MUI Training & Universal Precautions at the Lucas County Board of Developmental Disabilities every year. We provide online resources to our employees for continuing their education & keep up with the new rules and regulations regarding serving individuals with developmental disabilities. Mandatory meetings & inservices are enforced to be sure our staff understand all new rules & regulations as they arise.
u) Please describe your agency’s internal system for quality assurance.
Our First Love Residential Services require that all services provided to our clients (according to their ISP) is documented & reviewed daily. Any questions or concerns voiced from client/family or support team and/or administrator about inadequate service will be addressed the same day, no later than 24 hours.

We off companionship with our clients, we provide one to one services and develop lasting relationships by participating in different activities such as community outings and dinner. Our staff can assist with activities of daily living & teach homemaker skills (if needed). We make it our priority to get to know our clients so that they would not be just clients, but family.
