Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Hogan’s Care is a comprehensive human service agency whose mission is to empower individuals and families with diverse needs to realize their full potential for achieving meaningful goals, guided by principles of independence, wellness, and safety.

b) Describe your agency’s staff orientation and training process.

Each staff member will be required to attend the following training classes:
Introduction to MR/DD (Rights of persons with MR/DD)
MUI Rule
Specialized Support
Red Cross First Aid and CPR

In addition to the training classes, each new staff person will shadow(and be evaluated by) a trained staff person for two days.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

We share information and options with our individuals that empower them to select their services, agency staff and service outcomes.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Day Hab/ Voc hab services M-F from 8am-5pm. We have 24 hour suppport staff for all after hour services.
We offer 24 hour transportation
We provide services to all of lucas county and surrounding areas.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

In the event of an agency staff illness or emergency. Hogans Care will have on call staff to fill the position in case of absence.

f) Describe how your agency would address individual or family concerns.

If a ploblem were to arise, we would try our best to resolve the issue. We would arrrange a conference between the indiviual/guardian, agency manager and whom ever the concern was with. At that time we would discuss the problem and devise a plan of action.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Recreation and leisure activities are a critical dimension of the quality of life for all people, including those with developmental disabilities. We placed a emphasis on supporting individuals in a wide range of community recreation/leisure activities and settings, on an individualized basis. Which would offer greater opportunites for social connections and relationships. The agency provides transportation and accomodations so that all needs are met.

Hogan’s Care is a human development organization that focuses on Day Habilitation Services and HPC service. We create a fun, activity-driven environment where individuals with Developmental Disabilities can interact with each other and our staff in a safe environment. While promoting social development through various community based activities, we also encourage healthy habits through various exercise programs and nutritious meals.

We are more than just fun though; in addition to fulfilling the contract of each Individual Service Plan (ISP) we also provide opportunities for individuals to earn income by working in an enclave. In this setting, we teach valuable vocational skills and in effect create a higher level of self-confidence within each of our participants.

Hogan’s Care prides itself on creating an environment that is vibrant and technologically savvy with access to computers, video games, a movie room, and learning center. The environment nurtures the potential in our participants challenging each and every one to achieve their own personal standard of excellence. We help participants achieve their dreams. If one to one service are in aindividuals ISP, we offer that service. When a consumer may wish to do something special within the community, we will help the individual in doing so and this includes providing one to one services.

Our agency will go on a "field trip" at least twice a month. Some of the outings will include the visits to local parks, COSI, The Toledo Zoo, The Toledo Museum of Art, and other attractions around the city.

A few of the recreation/ leisure activities we will do include visits to the Toledo Library, The Botanical Gardens and the YMCA . The facility will also offer a variety of recreational/ leasure activities.

We have various activities that involve the consumers and the staff as a whole, such as a early event we have at the local college.

i) Describe a typical day for an individual who receives your agency’s day services?

A typical day starts with getting picked uup from home and getting transported to the facility. At the site you would have the choice to enjoy an array of individual and team activities. Some of the activities would include pool, video games, and ping ball. Also you would have athe choice of many learning acitvites, which would include reading a book, computer researching, etc. During the course of the day there will breakfast andl lunch also available. The day would end with getting transported back to your home.
