a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.
The mission of the Caldwell Adult Day Hab Program at the James C. Caldwell Community Center is to improve independence, productivity and inclusion in community life for adults with developmental disabilities. This is achieved by providing meaningful activities that are age- and developmentally appropriate and are goal directed.
b) Describe your agency’s staff orientation and training process.
All staff at the Caldwell Community Center have taken courses and certified by the Ohio Department of Job and Family Services. All staff is certified in CPR, First-Aid, Self-determination, Positive Culture, Quality Management, Developmental Disabilities, Individual Service Plans, Behaviors, Major Unusual Incidents and Therapeutic Assault Prevention.
c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?
Individual clients meet with all staff members.
d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).
The Caldwell Adult Day Hab offers services from 9:00 am to 2:30 pm Monday through Friday. All services are scheduled and implemented as agreed upon in the Individual Support Plan meeting.
e) Describe how services will be provided in the event of an agency staff illness or emergency.
It is incorporated in the Caldwell Community Center policy that staff cannot leave unless they have been relieved by another staff person.

Families are informed of the process for addressing concerns and who to contact during the Individual Support Plan meeting.

Clients are offered many community based activities. Each Individual has a support program developed around his/her interests.

The Caldwell Day Hab Program helps adults with disabilities to live full and meaningful lives. We work to discover strengths and interests, and then assist adults to realize their goals and dreams.

The Caldwell Community Center provides continuing education and on-going training to all DSP’s through regular staff meetings, and special training sessions. Topics include refresher training for Capability Teaching, MUI, CPR, First Aid, Health and Safety issues, and other topics of relevance to our organization.
u) Please describe your agency’s internal system for quality assurance.
Each employee shall receive a regular, written Performance Evaluation by his/her immediate supervisor. Satisfactory evaluations may lead to salary increases and may be a determining factor in promotions within the organization. Performance evaluations shall be a part of the personnel record of each employee.
Staff receive a performance evaluation that measures the employee in terms of fulfillment of job requirements as described in his/her job description, the extent of attainment of previously set objectives, and the setting of mutually agreed upon objectives for the next evaluation. Objectives are specified, measurable, realistically attainable and relative to the development of job skills. The employee participates in the evaluation process by setting his/her objectives and may freely express any objections or criticisms which are relevant and reasonable.

We help the individual in assistance and training to maintain and improve their skills in basic daily living, community integration and enhance their social and personal development.
Every area offers skills and socialization opportunities for the adult with developmental disabilities. Individuals have the choice to participate in any or all themes.

Activities are planned specifically for each individual to reach their particular needs and goals. The wide variety of activities help the person learn new skills, have new experiences and obtain the supports necessary to pursue their valued outcomes.
Activities are based on proven methods to maintain communication, self care, independent living skills, fine and gross motor skills and social behaviors.

We have a variety of recreational/leisure activities including ceramics, arts and crafts, computers, fiber arts, horticulture, movies, entertainment, pool tables and board games. The Caldwell Day Hab has a large gym for activities.
