Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

To provide the best quality of home care services to the disabled population. We want to be the top pick for our consumers. JTLCS will provide personal care assistance and other home care services at the patient's home. We want to be the best choice by healthcare professionals looking for a career and employment opportunities in the home care setting.

b) Describe your agency’s staff orientation and training process.

All staff are required to complete the 8 hour DODD training, and 12 hours of home care/homemaking education. Once office orientation is complete the new employee will have field orientation for one week at the individuals home.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

All individuals receiving services with Journey Through Life Care Services are included in selecting the staff that will work with them. Surveys are conducted every 6-8 weeks for feedback from the individual about our employee. Also, the individual will have the 24 hour oncall number to call about any concerns or issues.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

We are available 7 days a week 24 hours a day. We serve individuals in all areas of Lucas county.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

At JTLCS we have oncall staff available if a scheduled staff member calls off for an illness or emergency.

f) Describe how your agency would address individual or family concerns.

At JTLCS complaints or concerns from individuals and/or family is major concern. The concern will be addressed immediately by a supervisor and a response will be given to the individual and/or family with 24 hours. All individuals involved on that individuals care will be notified.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

We encourage our clients to get out and be involved in activities within the community. We offer HPC transportation services so that staff are able to take individuals out into the community. We will do what it takes to assist individuals out in the community including providing extra staff if needed, involving the individual in all the decision making when going out in the community and assisting the individual with any barriers that may be preventing the individual from going out into the community.

All staff will receive the 8 hour DODD training and 12 hour JTLCS office training upon hire and annually. All staff members are required to attend monthly meetings to cover any new information, rule changes, etc.

At JTLCS we review all documentation weekly and compare it reimbursement reports to be sure that services received and reimbursement are cohesive. The quality assurance checks are performed by the CEO; they are logged and placed in the CEO's office.

At JTLCS we put the individuals we serve first. We have an open door policy and every complaint is taken very seriously .We are great with communication; the individuals and/or families are free to contact our office at anytime The CEO has over 14 years of experience in the healthcare field; including but not limited to being an instructor teaching STNA's /LPN's and has always been a caretaker. We are very strict with educating our staff members and preparing them to provide top of the line services to our individuals. . We are very excited to provide services to you and your family.

JTLCS takes caring for individuals who require one to one services very serious. If an individual can not be left alone several staff members are assigned to that individual, oncall staff will be in place and the home will have a house supervisor to be sure the client is never left alone.
