a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

We are a Ministry Enriching lives and Individual Abilities

b) Describe your agency’s staff orientation and training process.

Staff go through a week of orientation in addition to having client specific training prior to working with any individual.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

In Day Hab each client is assigned to a person but clients have the ability to work in whatever group they prefer.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

We are involved in the process of discussing with the team on what an individual needs to work on or needs supports with.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

In additional to our Full-Time staff we also have staff that are considered Casual or work only as needed. These individuals have also gone through the orientation and client specific training process.

f) Describe how your agency would address individual or family concerns.

All concerns would be handled. Once brought to our attention we will gather all facts and share all concerns, responses or remedies with the team.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Sites/hours have vehicles and all staff are able to transport clients in their own vehicles.

Annually there is mandatory training required of all staff, med administration and recertification, CPR/FA and recertification, client specific training for all clients current and any updated changes, Health and Safety Alerts, Annual ISP information for all individuals served and any other training we see appropriate for staff training purposes.

u) Please describe your agency’s internal system for quality assurance.

Supervisors are on site during the entire program day. They are part of the individual's Team and they either meet or communicate on a regular basis to discuss needs, wants and/or concerns an individual has. During the program day the staff working with clients also discuss what they wish to do and also help them problem solve any issues and/or concerns they have relating to not only day hab but their personal lives.

We have 6 different sites located Lucas, Ottawa, Wood, Sandusky counties. Different outings consist of
Park, movies, ball games, hockey games, senior center, pet rescue, mall, restaurants, zoo, fire station, ect... We try to base the trips on the clients suggestions.
