Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Our philosophy is to treat each individual as if they are family and support their needs, desires, and abilities so they can become their best self. We want each of the people we serve to feel safe, accomplished, and cared for. We promise to treat each individual according to their wants and needs.

b) Describe your agency’s staff orientation and training process.

Preston Homes provides comprehensive orientation to all stall members including fire emergency and evacuation, facility layout and procedures, residents’ rights, first aid training, and training in correct techniques for providing personal care services. We follow all state rules when it comes to training that is required.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Individuals have complete control over who they want providing services to them. If their is an issue with the staff assigned to the consumer we are always open for suggestions and constructive criticism.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Preston Homes provides services throughout Lucas and surrounding counties. Services are available 24 hours a day, 7 days a week.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Support staff are available to fill in for any staff absences due to illness or emergency.

Preston Homes has an open door policy for families to express any concerns about the care of consumers. We maintain the privacy and confidentiality of all personal information.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Consumers will participate in stimulating community activities & social events. We will ensure that each individual is cared for in a dignified, compassionate and professional manner so as to reach an optimal level of independence and aid them in re-acquiring the life skills and ability to live in the community while maintaining their self-sufficiency, dignity and honor.
