a) Describe your philosophy of providing services and supports to individuals with disabilities.

I want to develop, achieve, and maintain a quality of life for people living with developmental disabilities and be impartial of their preferences and choices. I am learning through interpersonal interactions with my clients when they are experiencing personal difficulty and it helps to find positive meaning in an overwhelming event.

b) Describe yourself and your background/experience with individuals with disabilities.

I am a Vegetarian, and I practice Yoga. I love shopping off Ebay & Amazon for fashion jewelry, I like amusement parks and going to professional sport games. Having being a natural caregiver, I’ve been allowed to make personal assessments on the core values and if they are being followed. I respect my clients’ needs and take into consideration of their individual wants. I love to help those that are in need because it is a self-gratification. Compassion builds on what I know about helping because compassion is genuinely wants to understand or even help people. My goal is for successful outcomes that can be seen and the whole person is viewed as a unique addition to humanity.

Throughout my experience with working with an individual with developmental disabilities is to provide my clients with the same individual choices and control over their daily lives as non-disabled people and I provide my clients their equal rights, dignity, equal treatment, independent living, and full participation in society. I feel choices are important to people with developmental disabilities for this reason: Individuals with developmental disabilities want to decide what their own lives will be like, as much as possible. Care giving crystallizes what is important in my life and the love I have for helping others. I also developed team player skills and I speak up by expressing thoughts and ideas clearly, directly, honestly, and with respect for others and advocating for the client. I developed way to create opportunities for greater independence, enhanced skills, built self esteem, met each of my clients ‘individual’s needs, and engaged the community in the mission.

c) Describe any specialized training or education you have received to prepare you to work with individuals with disabilities. Describe any areas of expertise.

I am trained in implementation of individual client plans and team planning for people with developmental disabilities. I hold a current First Aid/CPR Certification, Medical Administration of Medicine Certificate G-Tube Certification, OSHA MUI Client Abuse & Neglect Developmental Disabilities, 101 Rights & Responsibilities, HCBS Waiver Services and a Chemical Dependency & Addiction Certificate. I have two AAS degrees in Medical Assisting/Medical Office Administration. I also have a Bachelors degree in Health Care Administration. I obtain an AFC (Adult Foster Care) certification and I am currently seeking clients to provide general supervision and personal care services for individuals who require minimal assistance in activities of daily living (ADLs).

d) Are you certified by the Ohio Department of Developmental Disabilities to provide medication administration?

e) Describe your availability and flexibility in scheduling services and supports for individuals with disabilities. (Days and hours available, areas of Lucas County you are willing to provide services, etc)

I obtain an AFC (Adult Foster Care) certification and I am currently seeking clients to provide general supervision and personal care services for individuals who require minimal assistance in activities of daily living (ADLs).

I will call upon other Independent Providers for support and assistance. If other IPs aren’t available to fill in, respite care services may be available in the community.

l) Describe how you would address individual or family concerns.

Family members may have an extensive knowledge of the person’s past history and could be a valuable source of information so I would contact the family to have a face to face meeting to address my concerns to incorporate our support for the best interest for the person with the disability.

m) Describe specific strategies for increasing community inclusion and involvement for individuals served.

I want to build a strong network of support that will help to link families with other families of people with developmental disabilities, participants, friends, neighbors as well as other supportive community members.

It is important for me to have a positive attitude and work together as a team and it is my desire to help others in need. I want to develop a way to create opportunities for greater independence, enhanced skills, built self esteem, meet each of my clients individual needs, and engaged the community in the mission.
