a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

To create an enviroment that allows an individual with developmental disabilities lifeskills that empowers the individual and make them more independent. Based on the individual skill level we create an enviroment that will allow your love one to learn daily lifeskills. This includes food preparation, with healthy meal choices, personal hygiene and social skills .

b) Describe your agency’s staff orientation and training process.

We have 13 employees (2 homes requiring 24 hr care) 4 employees per home 4 staff members at the day care facility 1 Transportation employee. All employees prior to being hired are required to complete first aid training, CPR, Rights of development disabilities, MUI, Specialized support,Unversal precautions and medication adminastration.We require our employees to participate in ongoing training and conduct monthly meetings for any updates.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

For our homes we allow the client to interview perspective care provider prior to being hired.

Available 24 hours all areas of Lucas County

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Each house has its a team leader and is on call 24 hrs. As for the day care we are well staffed with office employees who are also trained.

All concern are direct to the team leader of each house, if no resolution the team leader contacts the owner. If for some reason a solution can not be found a contact will be made to the individual's case worker.

The individuals at the daycare have outing 2 to 3 time a week in thecommunity.

We provide service for both Level one and I/O waviers. My staff is trained in first aid, CPR rights of development disibilities, MUI Specialized support, universal precaution nad medication adminastration.

Based on the individual"s ISP one to one service is provide. Presently the two houses we serve the individuals require one on one service.

Planned weely trips to the zoo, library, Imagination station, park and other outdoor activities. In class art and crafts movies and exersice equipment available to aid in motor skill.
