Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

T & Jewel Agape Love ,LLC. believes in a safe and nutritional environment where the individuals feel a sense of individuality and independence. Our focus is to create an environment where quality of life for people with developmental disabilities is paramount. We want to assist people with disabilities to have roles in their lives that will give them the dignity and respect they need from other People in the community. Our individuals will have equal opportunities to be independent regardless of their disability. Lastly we shall support the personal outcomes for our individuals. T Jewel Agape Love, LLC focuses on building self worth to empower people to live independently. We give our clients choices to enhance and to employ their interest which includes skill reinforcement, self-determination, recreation and leisure. We will assure all clients will work at his/her own pace identified in their ISP.

b) Describe your agency’s staff orientation and training process.

Our orientation process will take a full day. Hours will be from 8am-4pm.
Orientation will consist of getting welcomed from our staff members and owner of T and Jewel Agape Love.
Introduction on DD/Mission and History on the owner and how she got started.
We would want to discuss the rights of the individuals with disabilities
Next, orient the new staff members on the employee handbook and what is to be expected as a T and Jewel employee. We would also discuss payroll information/ time clock information.
Next we will discuss infection control; Universal Precautions.
During our orientation lunch will be provided.
We would then end the orientation with a tour of our facility.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Individuals with disabilities will have an input on which staff member will be involved in their care. If they have profound disabilities we will then gather information from their circle of support. Our agency wants to make sure we are involving the individuals with disabilities in choosing and developing the staff that supports them.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Our agency will have trained on-call staff in case of an illness or emergency.
If there is a possible emergency dealing with my agency staff member I will make sure the individuals guardian or family member will be notified immediately.

f) Describe how your agency would address individual or family concerns.

Our agency will assure our families/clients are satisfied with our services at all times. We would conduct a survey or have the individuals with disabilities evaluate our facility and get feedback on how we are doing, what needs to be changed or what does not. g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Our agency will have contracts with several local businesses to assure our individual stay busy and also employed. This will help them to continue to learn working skills, communication skills, knowing the importance of attendance, task completion skills, and dedication skills. These skills are vital to the individuals with disabilities growth.

Our agencies expertise is the fact the owner has been in the DD field for over 40 years. Ms. Murphy was employed at N.O.D.C For 10 plus years, being a Therapeutic foster parent for 29 years, and working in the Toledo Public School system for 18 plus years. She Also specializes in Behavior Modification. These experiences that the owner has accomplish has shaped her into becoming and advocate for individuals with disabilities.

The average length of service varies to the individual needs and to their ISP goals.

t) Describe your agency's plan for continuing education and on-going training for agency staff?

All staff shall complete their annual training and on-going training quarterly and yearly. Documentation will be kept on all employees regarding the training that was completed and when the training was completed. All staff training certificates are to be filed with the agency when completed in the employees file.

u) Please describe your agency’s internal system for quality assurance.

By the continuous monitoring, and assessing of staff members I can assure that with this method we can stay on top of any discrepancies in regards to staff and the agency itself. Staying on top of trainings and assessments will ensure continuous improvement and by doing this our agency will exceed national standards.

v) Include here any information about the agency that you want shared with an individual, family or guardian that will assist him/her to decide whether your agency is the best choice of provider.

Our agency takes pride in our work ethic. We also take pride on the well being of all our Individuals with disabilities. Our agency believes in letting our individuals with disabilities make choices and to have an input on there day to day activities.

During this pandemic, T and Jewel Agape Love, LLC are practicing safe measures to assure all family/guardians/individuals and friends stay safe. Temperatures are to be taken before any individual enters the building, and mask are to be worn at all times; unless any individuals are eating. Our agency also provides mask, gloves, and gowns if needed.

One to one services are offered to any individual with disabilities upon request. Our agency can offer this service to any individual who wants to go out into the community with another staff member and not in a group setting. This service is also provided to individuals with disabilities who have behavior issues. One to one interaction can sometimes help an individual with disabilities who has behaviors. This type of interaction can calm them down, get to the bottom of what’s going on, and also brings your focus/attention just solely on them.

Our agency offers planned group outings and activities for individuals with disabilities. Individuals will have an input on what activities they would like to participate in throughout the week. Some examples of our planned group activities are as follows; planned trips to the Toledo zoo, Art Museum, Botanical Gardens, and Mud Hen games. Our agency would love to get the individuals with disabilities out in the community to participate in a plethora of activities. Some examples of community activities are as follows; Fairs, Gardening, nature walks, crafting with clay, and painting.
Our agency will also have trained/certified massage therapist, and personal trainers at the clients/guardians approval.

We will have a lawn care service where our individuals can cut lawns, clean yards and plant flowers.
Our program focuses on attendance, task completion, problem solving, responsibility, communication and skill building.
We coach individuals in social skills communication practice work skills.
Food service-Menu planning, grocery shopping with a list.
House keeping, and organization skills are also taught.
Office-cleaning, sorting, filing.
Janitorial work, sanitation, safety skils included.
secretarial- Clerical skills, setting price points, shipping and more.

Toledo Zoo, Art Museum, Fairs, visiting public parks, Visit Nursing Homes, Attend a local Library, Picnics, Dancing, Karaoke, indoor water parks, attending festivals, grocery shopping after meal planning, Mud Hens baseball games, local basketball games, going out to restaurants, Movies, Bowling, Putt putt, attending the local Banks to learn money management skills.

Soft music, Dancing, Walking, Board Games, Sing A Longs, Karaoke, Instruments, Basketball, Ice Cream Socials, Parks, Yoga, exercising, story time, arts n crafts. We also allow our individuals to choose what recreation and or leisure activities they enjoy most.

Volunteer opportunities are accepted at T and Jewel agape love. Anyone who is in college and wants to specialize in working with individuals with disabilities are welcomed. Family members and or guardians are also welcome to come help and volunteer to assist in activities regarding there family member that has a developmental disability.
If we are looking in to community volunteers for our agency we can do so at Our local United Way Center. The center will welcome volunteers from the community while off work from the pandemic. These volunteers include, Teachers, factory workers, and retired individuals that are willing to share their time and passion for our clients.
