a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Tandoh Health Care Services (THCS) is committed to support, serve, and advocate for our clients. THCS values and respects each client for their uniqueness. THCS will continually strive to provide quality services and opportunities which enable personal growth, optimum healthcare, and independence so each person we serve may enjoy the life of their choosing.

Tandoh is a small agency provider because we believe services should be personalized and flexible.
Tandoh values and appreciates each person regardless of race, culture, or status.
Tandoh believes sometimes FUN should be spontaneous, especially if you are tired of everything having to be scheduled.
Tandoh management has over 20 years experience in the DD field.
Tandoh has experience with providing services for different levels of physical and intellectual capabilities; varying diagnoses; medical care; behavior supports; seizure support; just to name a few.

b) Describe your agency’s staff orientation and training process.

• Staff training plan

Staff receive training and on-going training in the areas specific to the individual. management routinely visit each individual home to ensure special accomodations are being met, and to ensure support for the individual and the staff. Tandoh Health Care Services has developed programs to train direct care staff to improve care of residents.

Training include; new entrant orientation training, medication aide training, ongoing in-service training, First aid and CPR renewal training, staff training on good care giving ethics.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

With Tandoh, staff is interview and hired based on the interest and needs of the individual. The individual and family/guardian would have an opportunity to meet and interview tentative staff. Staff are hired and trained specifically for the individual to ensure continuity and positive relationship building between individual and their staff.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Tandoh believe services should be personalized and flexible, it is Tandoh's plan to provide services only in Lucas County for more efficient supervision and direct managerial input and to meet the individual's needs even outside of routine schedule service hours. Tandoh specializes in providing 24/7 services, awake and non-awake staffing.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Tandoh has a strong line-up of direct support staff who have deep understanding of their responsibilities and would be willing to cover-up in cases of staff illness or emergency. Tandoh pays a time-and-halve for all overtimes unlike other providers, making staff eager to benifit from this extra payment any time their services are requested.

f) Describe how your agency would address individual or family concerns.

Tandoh firmly believes it is important to work with the individual's team and maintain good communication. Tandoh also believes good and timely communication with the individual and their team is important which we provide through calls, in person, e-mails and meeting. All individual, family and SaSS concerns would be followed up immediately without delays and feedback communicated back when necessary.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Tandoh is a member of People First, Civitan, IREC, and serves on additional committees. Through this, Tandoh receives a wealth of information on activities and opportunities available which are shared with the individuals we served. Tandoh continue to follow through with the individual on their interest in the activities of their choice. We believe it is important for the individual to have a say and for there to be follow through.

In addition to the 8 hours training required by the state, staff also receives training specific to individuals' needs, training on the individual's ISP and BSP's. Tandoh also provide misapproriation trainings to our staff about any misappropriation notification that comes out from the state. Tandoh also uses the opportunity to further educate and train our staff on some of the specific training programs by the board, example, training about "understanding and addressing challenging behaviors of individuals with complex needs, strategy and assessment".

u) Please describe your agency’s internal system for quality assurance.

Management and employees of Tandoh Health Care Services value and respect the resident’s right, culture and beliefs to self-determination. Tandoh Health Care Services will protect and promote the rights of each resident consistent with federal, state and local laws and regulations. It is essential that each staff person learn about each resident’s rights, culture and beliefs. The best way to do this is through proper staff training and implementing good care giving ethics.

1. Tandoh Health Care Services involve the individual in health/assisted care planning by ensuring that the individual actively participates in the decision-making process and is informed about alternatives.
2. Tandoh Health Care Services will advocate for the individuals interest
3. Tandoh Health Care Services will serve as a communication conduit between the individual and the family to ensure that the individual's wishes are carried out.

In the case where a resident does not have the capacity to engage in decision making, a consensus has been reached with the guidance/family and team on their right to self-determination.

Tandoh Health Care Services has ensured that individuals’ informations are handed responsibly and by only authorized personnel. All employees sign a confidentiality agreement with the company to maintain a strict confidentiality regarding individuals, conditions, diagnosis, treatment, or any personal information learned about them during the execution of duties.

Tandoh Health Care Services will be committed to full compliance with all applicable laws that govern individual’s privacy protection rules and confidentiality.

All transactions (purchases and payments) are authorized by management or appropriate person, and accurately and completely recorded. THSC and its employees shall follow the legal requirements of generally accepted accounting principles, established financial and accounting practices and completing the financial books, records and reports.

Tandoh Health Care Services and its employees will accurately bill for services provided. All claims submitted to the government or any other payors will contain the truthful information and be supported by adequate documentation, and reflect the services rendered.

Every noticeable incidents (intentional and unintentionally) have been reported to the MUI unit for follow up and advice. All employees are requested to report and document any incidents occuring during their shift, any employee who fail the report an incident during the shift is disciplined. Avenues are opened through the supervisors and management for reporting.

Tandoh Health Care Services investigate reports promptly and fully in a professionally manner without any prejudice and follow up with the MUI unit when needed. Incompetent and unethical conduct of any employee has resulted in a disciplinary action up to and including dismissal in some cases.

Individuals who perceived themselves to have a greater input into the decision of their up keep and service delivered are include in the decision that concern them. Tandoh Health Care Services will identify and implement conditions that will enhance improvements to service delivery and satisfaction.

New employees are carefully observed during the introductory period which include weekly performance progress report, tardiness, attendance and general behavior and conduct. There is an annual performance review of every worker and recommendations made based on self-improve job performances, satisfactory and unsatisfactory performances.

Tandoh Health Care Services performance evaluation will be based on surveys by federal, state and local agencies, and audit reports. Any unsatisfactory report in the evaluation will be considered a performance deficiency and will be corrected. Our first Compliance Review with the board came out perfect with no citation.

Tandoh does an excellent job in creating opportunities and empowering the individual to live their live and persue their interest. Tandoh is a member of Civitan, People First, IREC and other committees. Through this, Tandoh receives a wealth of information on activities which are shared with our clients and the individuals are helped to follow through with the programs and activities they want. Some of the programs and activities include dinner dance, special olympic bowling league, group attending mudhens games, hayrides, etc.

There has been individual in-home fish fry and barbecue parties where other individuals are invited for socializing and fun making, also, we arrange group day trips, example a trip to Amish Country Ohio where individuals have enjoyed seeing various ancient attractions and especially taking a buggy-ride which has continue to remain a memorable experience to them.

Tandoh curently has individuals who are active members of committees like People First and Civitan and we ensure they don't miss a meeting.
