a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Today’s Solutions Homes is dedicated to enriching the lives of the individuals with developmental disabilities. Today’s Solutions Homes believe personal choice is the most important consideration to determine all support services surrounding the life of every individual. Today’s Solutions Homes acknowledges the right of the individual, and promotes person centered planning. Person centered planning encourages and supports individuals with developmental challenges to be involved in all aspects of the decision making process regarding their ac-tivities and life goals, in order to achieve their maximum potential. It is the belief at Today’s solutions Homes, “within a community everyone belongs”. At Today’s Solutions Homes individuals with developmental disabilities will have the opportunity to live in a residence of their choice within the community, and live a meaningful and productive life in a supportive, safe, and secure environment, while maintaining their dignity and self-respect.

b) Describe your agency’s staff orientation and training process.

All staff will be required to receive all certifications required by the state. Staff will receive individual rights training, incident reporting, CPR, first aid, medication certification, behavioral support, and developmental training. Twelve hour training at home site with staff, individual, and supervisor to learn home routine and expectations. All staff must have a valid drivers license, insurance coverage, and a background check.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

Individuals can express their expectations and type of staff they require. Through staff interviews and training, we will determine who would be compatible with individuals needs and expectations.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Today's Solutions Homes is flexible with their availability. Would ask for time to review ISP in order to meet individuals needs.

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Today's Solutions Homes has on call staff for all emergencies. Management is also available twenty four hours. Management also receives direct care training and can fill in when needed.

f) Describe how your agency would address individual or family concerns.

will maintain open communication with individual and family to address immediate issues. A special team meeting can be held to address conflicts or special needs.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Individuals can identify community activities of interest to them using monthly calendar of local community events offered . Vacations and day trips, some trips are 1:1 to ensure individuals are able to have staff full attention and assistance in order to fully enjoy outing.

Today's Solutions Homes CEO has over 28 years experience in the field of developmental disabilities. TSH CEO has worked with all functioning levels as a direct care staff. Familiar with behavior plans and positive behavior support intervention. Also managed staff as well as provided training to staff on individuals rights, needs, safety ,developmental training, BSH plans, ISP, fire/weather/pedestrian safety, and money management plans. Ensure staff are compassionate, mature, and trained in all aspects of individuals needs to ensure all needs are met.

Agency CEO has participated in training with LCBDD Provider Support Liaison. CEO meets trainer qualifications to train agency staff. Ongoing monitoring, comprehensive staff training.

unplanned Supervisor visits to home, documentation of any unusual incidents, communication log for staff, and documentation on individual's programs.
