Agency Profile

a) Describe your agency’s philosophy of providing services and supports to individuals with disabilities.

Triad Residential Solutions provides residential supports and services to individuals with developmental disabilities that are driven to achieve positive outcomes emphasizing the individual's self-determination, dignity, and personal development. Respecting that each person is an individual, Triad provides choice for opportunities, securing the individual's role in the community.

b) Describe your agency’s staff orientation and training process.

Triad meets state regulations regarding staff training, as well as expects a minimum of 12 hours of training at a home site for each staff member. We offer orientation training at the start of hire that includes individual rights and incident reporting. We then offer trainings that include behavior management, history of services, and an introduction to the field of developmental disabilities.

c) How are individuals with disabilities involved in selecting the agency’s staff that work with them?

We offer the individuals that we serve opportunities to instruct us as to what they are looking for in a staff person. We expect through our training process that we will determine if the staff and the individual they serve would discover if they are compatible.

d) Describe your agency’s availability and flexibility in scheduling services and supports (days and hours available, areas of Lucas County willing to provide services, etc).

Triad Residential Solutions is able to offer a variety of scheduling due to the size of our agency. We have access to a large pool of employees, as well as the ability to hire and provide the requested staff. We are able to provide services 24 hours a day, 7 days a week. We can provide services almost anywhere (we currently provide services in Lucas, Medina, Lorain, and Cuyahoga counties).

e) Describe how services will be provided in the event of an agency staff illness or emergency.

Triad Residential Solutions provides a very clear chain of command that will ensure no individual is left without the appropriate supervision. We have a main office, as well as company-provided cellphones to ensure that there is a management person available at all times to handle potential emergencies.

f) Describe how your agency would address individual or family concerns.

Triad looks forward to feedback received by the entire Team, which includes the family and the individual. We always look for opportunities to improve and would like to know if we are meeting all needs.

g) Describe your agency’s specific strategies for increasing community inclusion and involvement for individuals served.

Triad has an activity coordinator, as well as a monthly activity calendar that is distributed to offer anyone an opportunity to get out in the community.

Triad offers annual training for all of our employees as well as individual specific training for all individuals we serve. Triad offers training for their supervisors specific to leadership annually and as needed.

u) Please describe your agency’s internal system for quality assurance.

The purpose of this internal quality assurance procedure is to simulate the review process conducted by the Ohio Department of Licensure, improve overall service delivery to individuals and employees at Triad Residential Solutions.
The focus will be based on observations, interviews and surveys of individuals, employees and guardians at Triad Residential Solutions.
Reviews will include the service area HPC, ADS/VH, Transportation, UI-MUIs, Human Resources, and individual/guardians.
Reviews tools will include Individual ISP service review, HPC services review, environmental review, ADS review, Transportation services review, Employee Development review, Unusual & Major Unusual incidents review, and financial review.
Samples will be taken from each area and a review will be conducted to ensure that services are being delivered as defined by the Ohio Department of Developmental Disabilities. Additionally, random sample surveys will be taken based on satisfaction of all areas provided at Triad Residential Solutions.
The Community Liaison &Quality Assurance Coordinator will oversee the implementation, monitoring, and follow up of this process. Reports will be generated upon the completion of each review.
There will be management oversight of all reviews and follow up as necessary in their respective areas in written format.
Each area will consist of a documentation form to be completed at the time of the review. The manager of the area as well as all other managers within the organization to be copied on the review.
The UI-MUI Reviews will be completed on a monthly, semi-annual, and annual basis. A report will be generated to meet requirements of both County Board and Ohio Department of Developmental Disabilities. Each report will be include review for any trends/patterns and any follow up necessary. See UI/MUI protocol.
Reports will be made available to County Board and Ohio Department of Developmental Disabilities as requested.

Triad provides an individual experience for each person they serve, they become part of our family and our employees ensure that each individual is part of the community and gains full potential. We host a guardian forum quarterly where we encourage family/advocate involvement. Triad has an open door policy, where every administrator is involved in the day to day operation and is available anytime.

Triad purchases season tickets to the Toledo Mud Hens and the Toledo Walleye. We also offer picnics, dinners, dances, and other various planned activities throughout the year.

Currently, Triad offers employment through confidential document shredding, selling arts and crafts, cleaning services, and envelope stuffing for local business. Triad has enclave opportunities such as working at the Toledo Food Bank, Toledo Grows, The libraries, Toledo Metro Parks and on a trash route for the elderly in the community.

Triad frequently offers activities and outings in the community, such as a monthly movie at Maumee Indoor, trips to the park, Toledo Zoo, Humane Society, and various other locations.

The courtyard at our ADS Facility provides an opportunity grow plants for projects and also allows for various sensory stimulation. We also provide educational activities such as Math and English either on the computer or on paper. We have multiple indoor games and activities which can provide enjoyment and entertainment.
